Glucometer Buying Guide

Picture: Diabetes Blood Sugar from Care Touch

Diabetes affects more than 30 million people in the United States, approximately 10% of the population. It is a disease that occurs when your blood glucose, or blood sugar, is too high. Glucose in the food you eat enters directly into your blood stream from your digestive tract. Your pancreas makes insulin, which helps that glucose get into your cells so the sugar is used for energy. When the pancreas doesn't make enough, or any, insulin the glucose doesn't reach your cells. It is also possible that your pancreas makes enough insulin but your body doesn't use it well. Over time, too much glucose in your blood can damage the tiny blood vessels in your ears, eyes, kidneys or nervous system. Symptoms of diabetes include excessive thirst, frequent urination, lethargy and hunger. There are three major types of diabetes

  • Type 1 Diabetes
Type 1 diabetes is an autoimmune condition in which the body attacks it's own pancreas with antibodies. The damaged pancreas does not make insulin. People with Type 1 diabetes must take insulin every day in order to stay alive. For this reason it is called insulin dependent diabetes. Type 1 diabetics must test their blood sugar levels at least three times per day. This testing helps them determine how much insulin they need to take.
  • Type 2 Diabetes
Type 2 diabetes is the most common form of diabetes and is often a more milder form of the disease. Type 2 diabetes can still cause serious health problems. With this type of diabetes the pancreas produces some insulin. But either the body's cells are resistant to it or the amount produced is not enough for the body's needs. It is possible to control Type 2 diabetes with nutrition, exercise, weight management and sometimes medication. The effectiveness of this regimen is monitored by a blood test every 3-4 months. This test, called the A1C test is performed by having blood drawn at your doctor's office and tested in the lab.
  • Gestational Diabetes
This type of diabetes affects women during pregnancy and is usually controlled with exercise and diet. Approximately 15 to 20% of women will need to take insulin to manage gestational diabetes. Otherwise, the mother's high blood sugar levels will be circulated through the placenta to the baby. It usually resolves itself after pregnancy. However, up to 50% of women with gestational diabetes may develop Type 2 diabetes later in life.
  • Other Types of Diabetes
Monogenic diabetes is a genetic disorder that is usually inherited from one or both parents. About 1 to 4 % of all cases of diabetes are monogenic. Cystic fibrosis related diabetes is a very rare form of the disease. Cystic fibrosis may cause scarring of the pancreas, resulting in the body not using insulin efficiently. These conditions may require the patients to regularly monitor their blood glucose levels.

Types of Blood Glucose Meters

For diabetics who need to monitor their blood glucose levels multiple times per day, the advances and innovation in development of these devices has seemed very slow moving. Early devices used to require the user to draw blood onto a test strip and compare the results using a color chart. Your doctor or diabetes educator will be able to help you determine which type is best for your situation

  • Digital Meters
Handheld digital glucose monitors have been the standard since the early 1980's. These devices are battery operated and require the user to have a supply of alcohol preps, a lancet to prick the finger and test strips. The monitor will then give you a readout of your blood sugar level. You will need to consult a chart to determine how much insulin you will need to inject in order to maintain a healthy blood sugar level. You will need to know how many carbohydrates you have consumed or are about to eat in order to accurately determine your insulin requirements. The monitor and strips usually come with a carrying case or pouch but can also be carried in your pocket. You will need to keep track of your readings and the amount of insulin you take using a chart. This information is important to bring to your doctor when you have your checkups.
  • Glucose Meters That Transmit or Store Your Data
Memory options for storing and retrieving data are available on many devices. Some are sold with USB plugins and cables so that you can download the data to your phone, laptop or computer. Recent developments in the industry allow meters to directly sync with an app, website or cloud to automatically store your data.
  • Talking Meters
Abnormal blood sugar levels may cause distorted or blurry vision. In some cases, diabetes can cause blindness. Many of the talking glucose meters on the market still require some vision to be able to operate them. But there are a few that talk you through the process. Look for a talking meter with volume control and/or a headset. These will allow you to test yourself discreetly without announcing your blood sugar levels to the rest of the world.
  • Continuous Monitoring Devices
Blood sugar levels can vary dramatically throughout the day. Finger stick testing provides a snapshot of your levels at the time of testing. Continuous glucose monitors consist of a sensor that is inserted under the skin and a wireless monitor that can be clipped to a belt or kept in your pocket or purse. The sensor monitors your blood sugar level at intervals of 1, 5, 10 or 15 minutes and sends the readings to the monitor. The monitor records the data to help you determine the amount of insulin that you need to take. The monitor will sound an alarm if your blood sugar is too high or too low or if it changes dramatically over a short period of time. The censors need to be changed every 5-7 days. You will still need to do a traditional finger stick test 3-5 times per day. Other continuous monitoring devices are under development. A very promising device consists of a glucose sensor and a very tiny antenna sandwiched between two soft contact lenses. The antenna transmits a signal to your smart phone to inform you of your readings.

Factors to Consider When Choosing the Right Glucose Monitor for You

  • Insurance
Your insurance company may have a pre-approved list of meters and providers that are covered under you plan. You should also check to see if they cover the cost of additional test strips, lancets and a back up meter.
  • Doctor Recommendation
Your doctor, nurse, pharmacist or diabetes educator will be able to steer you in the right direction as to what type of meter is best for your individual situation.
  • Accuracy
Accuracy is extremely important when taking blood sugar readings. Search for your meter at online stores or consumer websites. These sites will show you user reviews and ratings. Ratings will typically be on a 5 point scale (most sites use stars). You should choose products with ratings of at least 4 out of 5 stars.
  • Ease of Use
Some meters ae difficult to use, have slow start up and take a long tome to read levels. Others require frequent maintenance. Again, look at the user ratings to see what highlights and downfalls that current users experience. Is the screen easy to read? How difficult is it to sync with your technology? Does it only transfer to Android compatible products? Is it easy to insert the test strips? How much memory is included for data storage?
  • Cost
Digital meters and meters that can be connected to another device for storage can be found for $10 to $100. Continuous glucose meters may run $1000 or more. When considering cost, keep in mind that it is always a good idea to have a back up meter in case your original breaks down. Also remember to take into account the cost of buying more test strips and lancets.
