What password manager do you use?

There are so many password manager products out there that it’s hard to discern quality and popularity by hearsay and articles alone. Polling the community will help narrow down the choice-overload and give those in the market for a new manager a starting point for their research.
Please add any manager that you use that I forgot to add, specifying whether it’s hosted or self-hosted, if applicable.
